Information that helps us assess your housing circumstances

Please tell us:

How your current accommodation affects your medical condition.
What difficulties you experience in your home because of your condition.
Whether there are features of the property that make daily activities difficult.
How moving home would improve your circumstances.
Whether a health professional has recommended a particular type of accommodation.
